The Population Health module introduces participants to impact of population health as a bridge between traditional medical care and public health. Competencies of population health are addressed in each of the submodules.
This module consist of 5 sub-modules. The module takes approximately 1 hour to complete and you may complete each sub-module at your own pace.

Module 1: Overview - What is population health?
Module 2: Public Health - This sub-module develops the ability of the participant to assess characteristics within a community that lead to increased risk of disease.
Module 3: Team Skills - This submodule goes beyond collaboration to identify where dysfunctions may occur within a team and how to resolve the dysfunction.
Module 4: Critical Thinking - This submodule promotes the use of data, qualitative and quantitative, to implement a quality improvement project to address population health system issues.
Module 5: Non Traditional Partners - Interview with FDNY Fire Chiefs - Chiefs Suriel and Woznica discuss the use of Geospatial Information Systems (GIS) Maps: e.g., drug, alcohol, and opioid abuse in New York City boroughs. Learn how to apply these to your practice.

Evaluation - Please complete this brief evaluation to receive CE credit. 60 minutes spent on completing these modules is equivalent to 1 CE. Total CE credit earned is proportionate to total time spent (e.g., 30 minutes = 0.5 CE; 15 minutes = 0.25 CE).
